To request a copy of your medical records, please contact the Stopwatch Urgent Care location where you received care or email your request to [email protected].

Please include the information necessary to identify the patient and the records being requested. A valid authorization may be required before records can be released.

Attorneys, Law Firms & Record Retrieval Companies

Requests from attorneys, law firms, and medical record retrieval companies should be submitted through ChartSwap.

Please visit www.chartswap.com to submit your medical records request.

This includes requests from plaintiff or defense attorneys and third-party record retrieval companies. Appropriate patient authorization and supporting documentation should accompany the request when required.
